In the Manga, Brendan actually does use the hat to hide a large scar on the top of his head that he earned defending someone else from a Salamence attack. In the games though, the hat never gets removed until ORAS where Brendan wears a different outfit for contests.

I think the artist just sucks at drawing scars, or just didn't want to make them look too graphic, which is still different from "censoring".
