They did, Satoshi and Shigeru were commonly used names for them. It wasn't until gen 2 Red and Green were made their official canon names.

Sugimori has confirmed the girl in that picture is just some random girl he added to it because he needed someone to have the third starter, she was never meant to be in any games.

do you really think this would change something? the international localizations would still give everyone another name for every language. pokémon isn't like any other franchise

They didn't want the 3rd starter to have a trainer so you don't get to register it in the pokedex, forcing you to buy the game again to start with that starter or beg your friends to trade you it (and they knew nobody would trade their starter)

they probably scrapped the Oak's battle for the same reason, to make people who want to complete the dex to buy the same game again
