Historically many men in Japan would also go commando if they didn't expect to ever undress before the end of the day, but Japanese men regularly did physical work, even in high society, so that was rare.
There were women who did wear fundoshi and it wasn't considered taboo or unusual because they worked in riverine and coastal industries in which they would have to take their kimonos and hakama off as part of the day. Those same women would go commando on the days where they weren't working.
