>>46638061 proposes. And because Almia has a snowy mountain range in the north, it can't be connected on the east either. So, it makes the most sense that Almia's mountain range connects to the Snowpont area.
>Kalos and JohtoHe bases this on two things. First, he actually looks at the Pokemon available in the games and finds that a lot of the Pokemon found in the west of Kalos can be caught in the east of Johto. Second, according to X and Y, Sycamore apparently arrived to Kalos by taking a train from the Sinnoh region through the mountains as he was once Professor Rowan's assistant. So, if the Sinjoh ruins are midway between Sinnoh and Johto, this means that they share a landmass/continent. Thus, Kalos must also be a part of this landmass in a way that would allow a train to go from Sinnoh to Kalos.
The map isn't perfect, but there was a good amount of research put into it.
