Pokemon attacks are magic and different from the actual thing. It's the same reason why Pokemon that have a weakness to water attacks are unaffected by regular water.

at the very least, in gen 1 this was not the case, as that one gym dude had his sandshrew practice diving into a pool to toughen up and practice resistance.

There has always been a distinction between Ghost type Pokemon (some of which are former humans reborn as Pokemon, but still living beings), and actual undead ghosts.

Because Charmander was already weak due to reasons unrelated to the train. The Pokedex says that rain doesn't affect a healthy Charmander's tail.

Origin's also showed Red's Charizard's tail flame stay lit underwater

Back then, the anime was more free to expand on things, unlike nowadays where it's a carbon copy of the games. Charizard's tail being connected to its health didn't contradict anything from the games, so yes, it was canon.

It was mentioned in the games in Gen 2.

It doesn't work like that. Games adapted this to their canon, but not the fact that the flame is so simple to put out.
