Actually the first sprites of Machamp had some details at the waist, which carried over to the revised Blue version art. GS would do the same, but it would be Crystal that first erased the belt details. They'd remain gone through all of Gen 3, but return for Gen 4.
That Machamp in all its animated appearances and its Stadium model had the belt heavily imply the belt was always intended and the instances without it were the error, not the reverse.
