Ho-oH was the only one in(what eventually became) Gen2 and was made in-house by GF(hence it being the one that showed up in anime ep 1). Lugia was later made to push a movie and had to be forced in the games. They weren't trying to "make sense" in lore, it just so happened the 2 weren't related at all.

Its entirely unrelated to how many/which universes are there, if any. So its irrelevant whether a pair of games in inconsistent. 99,999999% of the time people use it they actually mean "continuity".
Universe is the space-time continuum in which the events portrayed in the product(which can depict more than one universe) take place in. It has nothing to do with canon and pokemon does have a confirmed multiverse so even then its entirely possible for both versions to count.
