Trevenant and Kantonian Persian are the only ones that meet the requirement.
Kantonian Meowth only evolves into Kantonian Persian, but it is not a regional form.
Sirfetch’d goes into regional form evolution and Galarian Linoone in regional form.
Branched evolution
You can make the case for all of these being branched. However, Kantonian Marowak is only a branch in USUM when the opportunity for Cubone to evolve into either exists.
Cross generation - You are right on the money on this, but Sudowoodo has more in common with Wobbuffet and Marill. Cross gens may be separated into: cross evolutions and cross pre evolutions. Roselia is the only Pokemon so far to unquestionably have both. (Galarian Mr. Mime may count but it was introduced in the same gee as Mr. Rime.)
Regional form - Everything here fits.
Regional evolution - They are all regional forms as well, but the distinction is that their pre evolution does not have a regional form. Alolan Marowak fits here.
Regional form evolution - Everything here fits but Sirfetch’d belongs to this category as well. Consider it more like Cursola where its regional form (Corsola) was derived from a species that cannot evolve otherwise. If Cursola fits, so does Sirfetch’d.
