During the opening cutscene for Black and White you can see Ghetsis's right arm, and it's not only extremely discolored but also has scars. Concept art that's been released of Ghetsis shows that his right eye also suffers from the same discoloration/scarring. His right arm also seems to be of little functionality (at one point when he claims he'll beat the PC, he says "with my own HAND", rather than "Hands"), and he indeed also has trouble using the right side of his face.
Now, keep in mind that Ghetsis is known to absolutely despise Pokemon and is confirmed to abuse them to get the performance that he wants, his signature Pokemon and ace is a Hydreigon that is famously underleveled, and that it's been proven canon in games that Pokemon can be forced to evolve before they're ready for it through extremely stressful or painful circumstances. It's also canon that Ghetsis has studied and researched all past Villainous Teams before him (most notably and importantly, Rocket and Galactic) to see what they accomplished and where they succeeded while learning from their mistakes. Also remember that Ghetsis's Hydreigon in Black and White 2 is even more underleveled than the prior one he had before and has frustration at maximum power, meaning that it (and by extension all of his Pokemon probably) REALLY fucking hates him. And Hydreigon as a species are extremely violent and murderously insane Pokemon even before such things are taken into account.
Essentially, the implication is that Ghetsis forced his Zweilous to prematurely evolve into a Hydreigon, and in the process it went absolutely ballistic and completely mauled him before he got it under control (probably by beating it with his fist or cane). He got an extremely powerful asset under his command, but as a result the right side of his body has been horrifically scarred, rendering his right arm and right side of his face mostly useless. It's possibly he's even blind in that one eye.
