At this scale you can use json files for saves instead of whatever preprogrammed binary shit pokemon games use, and the problem is now solved

Nah, for that you just need a version number in the save and migration code that indicates what to do when upgrading from version x to version y
When the new game version comes, it checks your save's version number and gives you the item if it's older than when the change happened

Yes, this is absolutely possible, yet I haven't run into many fangames that do any such thing. More often they note that old saves won't work with the new version and advise players to delete their saves when upgrading.
