We already see in Alola how that specific league was chosen but that's a special case.
Apart from that we don't really know but probably something to do with being able to hold their own against the current champion and other Elite 4 members.

Volkner was popularly strong he got asked to be part of the Sinnoh E4, but he rejected the proposal.

Sydney was given the spot after turning a new leaf.

Kukui being a lazy fuck picked the guardians, Nanu said no, Acerola was close, Kahili was nepotism.

I would assume the champion's at least in the older generations probably picked the Elite Four, that would seem to be the case for the Kanto/Johto and Unova Elite Four. I think Steven and Cynthia are probably too young to have been personally responsible for the Elite Four beneath them aside from maybe Phoebe and Aaron, Flint probably became an Elite Four around the same time as Volkner and might've originally been the Sunyshore City Gym Leader before Volkner similar to how Koga rose from Gym Leader to Elite Four.

I'd like to think that the Kalos Elite Four represent the three sub-regions of Kalos with Malva representing Lumiose City since it's by far the largest settlement.
