Is it possible for the gameboy to actually do picrel? I chose this one because while others have a lot more detail in them, this one feels more faithful to the simplicity of GSC.

If you're talking about the very first GameBoy. Absolutely not.
The GameBoy Color might not handle that many colors ironically. More so, there are many NPC to run independantly.
The GameBoy Advance could run something like that.

In terms of lore and worldbuilding, how do you justify your region having only fakemon?
Hardmode: no "very far away from the canon regions" or alternate universes

clover explained it away that the fakemon were fabricated by meme magic from clovenix (basically they aren't pre-existing species that were not discovered they really are new pokemon).

maybe you could take an approach similar to australias wildlife laws (they are super strict on introducing new species so its mainly just native wildlife in aus)

You could do an approximation of that using sprite overlay tricks, but I can tell you now you wouldn't have a lot of space in the tileset for much else outside of that city.
If you wanted to do a Gen 2 style in Gen 3 deal, you could absolutely do that.
