If a Hisuian form pokemon falls into a paradox portal then would it just become the modern day Sinnoh version of that pokemon?

It's still the exact same Pokemon, time traveling isn't going to cause any physical change to it.

A caveman wouldn't suddenly become a modern day human if he got time warped to the present.

Yeah. And they weren't even predecesors of their "classic" forms. Classic Typhlosion, and Qwilfish existed outside Hisui at the time of PLA, so the Hisui mons aren't ancestors.

Also why it'd make no sense for Weavile to be modern Sneasler, the regular Sneasel/Weavile were already in other regions.

The "regular" forms predate the Hisuian ones. The Hisuian forms are forms they adopted they migrated to Hisui.

Basically most forms are confirmed to coexist at the same time as their classic/traditional/whatever.
