Prism, Clover, Gaia are all excellent romhacks with new regions/stories and are harder than main games (Clover is a lot rougher)
As for fangames, Reborn is challenging/good, has a bit of a cringey story but it's one of if not the most expansive and exploration heavy Pokemon fangames out there.
I'd also recommend Ashen Frost, Infinity, Daybreak, Realidea System, Tectonic and Opalo. You can also try out Rejuvenation, Reborn's sister game, while it isn't finished it has a lot more content than two regular games put together
