Given how readily they were recycling old bosses for Gen 2's E4, I would be perfectly content to believe they had both her and Karen on the table and went with the latter since she was both a new design herself AND used a new type. Bruno having a third hitmon to show off and absolutely looking the end-boss part in the 90s didn't hurt. But for all of that, they could have found to a place to stick Lorelei and Agatha even if you couldn't battle them yourself, yeah. When ever other boss from the first games are there barring the obvious Giovanni it stuck out when they weren't.
