This can be things as simple as mild autism or learning difficulties if the tantrums are bad enough, not just stuff like cerebral palsy or downs syndrome. They may visit them on occasion, but some just let them rot there and the kids don't get educated beyond the very limited capabilities of the home staff, who aren't exactly qualified to teach. Naturally, they can't function in society with how little they know and the best they can hope for is eternal NEETdom, with their families rarely having anything to do with them. The unlucky ones just go from one assisted living home to another for their whole lives, with their families paying to hide the shame.
A few years ago now, a loon killed 19 people in one of the adult assisted living homes, claiming it was to improve the nation as disabled people are holding Japan back. Only 1 family claimed their relative, which they need to do to get put on the family grave plot, so the rest of them won't even acknowledge their relative in death.
I'd take a vacation to Japan for sure, but the way they're wired socially disgusts me.
