Even blatantly overpowered Pokemon like Shaymin-Sky, Palafin, Zygarde-Complete, Houndstone, and Marshadow had their time in OU. That means every time Game Freak drops a new Pokemon, it starts in OU even if it's a shitmon like Charizard, Decidueye, Samurott, or Typhlosion.
This is also why every new generation only starts with OU, Little Cup, and Ubers as tiers. It takes 1 month for UU to form, then another month for RU, then another for NU, and so on. When Crown Tundra dropped in Gen 8, 100+ Pokemon were in OU.
