- It would have been more like Gen 1 in terms of gameplay
- A lot of Pokemon would be different in design
- "Johto" as we know it would not exist, in fact, this would spread into Gen 3 and 4, as the entirety of Japan has now been adapted as a Pokemon region and can no longer be revisited without looking redundant
- Story would be very different and completely unrecognizable, Silver would likely be much more like a Blue clone.
- Sneasel would be a mythical Pokemon, this means that neither Weavile or Celebi would not exist. This extends to multiple cross gen evolutions like Overqwil or Sirfetch'd
- Friendship evolutions would not exist
- Evolution items would be more prominent in exchange with things like Poison Stones and Heart Stones
- Typings would be different
- Cyndaquil would not exist, this means nobody will complain about Typhlosion's fire ever again. I'm mentioning this because this might actually be the best thing to come out of this hypothetical scenario
- Kanto as we know it in GSC would not exist, making it more in line with future Pokemon releases
- Game likely still would have been a success, but I have no idea if the momentum would carry into Gen 3 or if it would have died releasing so soon after Gen 1 and it being so similar
- Based Stantler would not exist, meaning the franchise would have likely died after this release
