Were Mega Evolutions a bad idea from the outset? Or could it have been something cool if they didn't botch the designs 90% of the time?

Shame, but given their 2nd iteration in ORAS included things like M-Salamence and M-Sharpedo, odds are it was only going to get worse if they made the mistake of keeping it.

Yes, mainly because of the way Game Freak implemented them, because the only though
Either Pokemon that didn't need one got one
Pokemon needed it and got one but it couldn't compete with the broken shillmon megas
Pokemon needed it and got it AND they actually got to have value for once, but end up being completely reliant on a gimmick form that's gone now, and likely wont even receive a proper evolution so they're stuck as worthless shitmons forever.
It was a poorly thought of farce that only existed to shill CHARIZARD

Yeah, M-Ray is pretty bad. Randomly add ribbons to overcomplicate an otherwise pretty simple and appealing design.
I think the design of the Pokemon matters a lot though. Even if you like the "hurr durr, make it stronger" button, it's still a more often than not "make them ugly" button too.

roughly a third of the designs in worse are at least equal to their base design, but megas were ultimately a mistake since there are now a ton of shitmons carried by their mega (or a similar form change mechanic, like necrozma) that will never again be useful but also can't get evolutions because gamefreak likes to randomly put the fucking mechanic in spin-offs

That third is likely in the "Added Absolutely Nothing" Category.
Like, I guess Mega Venusaur is technically equal to regular, given that it's just Venusaur again, but it does nothing to justify its own existence.

They're not a terrible idea, if we assume that Gamefreak is never going to make the Beedrill you can get at level 10 before Brock capable of going toe-to-toe with actually strong pokemon in its base form. They just need to do more Mega Pidgeots, Mega Beedrills, and Mega Gardevoirs, and less Mega Mewtwos and Mega Salamences.

Mega Medicham was also an improved design. Mega Buneary too.
Mega Scizor was an okay as a super-mode design, as was Alakazam
