The same way that the player saw the professor at the Crystal Pool. Terapagos can temporarily transport people/Pokemon across space/time, it transported paradox Pokemon into Area Zero when Heath was on his expedition.

He didn't the first time, his team got attacked by a Donphan and they made a sketch based on it, which led to the creation of the Paradox form by Terapagos. It's also the reason why this was the only paradox seen in the expedition and why only one was in Paldea.
Before you come up with schizophrenic time travelling crap or even attempt to say there were more paradoxes, reminder that Area Zero was protected by a barrier ONLY at the time of the professors going there. There would have been a lot more instances of paradoxes escaping to Paldea in the 200 years without a barrier.

They also saw the paradox legendary beast/sword of justice. The Occulture magazines also imply that the other paradox Pokemon were also described in the Scarlet/Violet book.

It was never explained explicitly. We only got pieces to come to our own conclusions:
One say that during the battle with the AI, Sada's/Turo's time machine accidentally dropped some of the Paradoxmons onto Heath's expodition within the great crater of Paldea.
Another say that Terapagos powers allows Heath and his team to capture glimpses of the Paradoxmons.
Either way, there's no concrete answers; only headcanons.

Heath initially thought that his own encounter where he was teleported by Terapagos was also just a hallucination. It's only when he saw the manuscript with his own handwriting that he started to question the reality of his experience. There's a good chance that the artist also had a similar experience where they were transported and saw the paradox beasts.

False, he specifically mentions it being like a dream, he didnt think it was a hallucination, he wasn't even conscious.

I already attempted to explain how the Tera type changes actually work, I want to believe it has a foundation on crystalized poison being imbued into Pokemon which resonates with the Tera Orb's concentrated energy that allows for crystalization. Not unrelated to the way he generates Tera energy, as the poison seems to be generated within the process of converting energy to Tera crystals, likely a process of radiation to explain the Terrarium

The pictures in the ScarletViolet book aren't even the same as the ones we see. They are different, and the new paradoxes aren't even referenced from the book except Wake and Boulder.

I just have speculation, no answers.
Is it possible that terapagos transported heath to the timeline where the paradox Donphan was real and they returned after getting a picture of it? I'm not seeing anything to immediately support the idea, but given how the professor just sort of appeared out of the mist maybe something similar happened to heath and party, which is why they wouldn't have realized they were transported.
Speaking of the mist, I wonder if there's any connection to terapagos using mist and area zero crator being completely covered in clouds.
