Why was Koga promoted? In gen 2?

What's the point of making second Poison type trainer? They could have Ground specialist instead because GS is missing Giovanni.

Johto gym leaders kinda follow the pattern than next one is weak to the previous one, but that's it?

Flying > Bug = Normal > Ghost > Fighting > Steel > Ice > Dragon
Psychic > Poison > Fighting > Dark

Gym leaders needed to add Dark type which loses to Fighting or Bug (no chance for E4 type), going further Fighting loses to Psychic, Flying and Poison.
Even if they just needed Poison type for this pattern, it would work better with the missing Ground type instead of another Psychic because it still beats Poison.

The whole point was to build on the world more and show you that Gym Leaders can advance to Elite Four and such and it's not just static set dressing for the gameplay.
Though I always wondered why Blue got demoted to Gym Leader, shouldn't he be the fourth Elite 4 member?

That's unironically why.
Gen 1, the elite 4 had a much lesser affiliation with their type, and more of an affiliation with a particular strategy. RNG lottery with Agetha, Lorelei trying to stall (also defensive ice type specialist, lmao), Bruno... well, TRYING to be the raw power trainer.
Koga fit that theme best, since while he's on paper the poison guy, in practice he's about indirect combat.

It wasn't until gen 3 that the elite 4 just became 4 more gym leaders.

Yes, except Lance.
Lance was already the champion in gen 1, presumably he just took the position back once Red left it, since it's not like anyone else wanted it.
