You've set out on your journey as a young trainer and you can have 2 named NPC characters from ANY of the games (or anyone from the anime who isn't a PC in the games) join you on your journey after encountering them along the way.
The region you decide to start from decides who can join you (spin-off characters are limited to the main region of the generation their game came from). You can have someone from another region join you, but only after you've traveled your starting region first. Once you've gotten to your second region, those will be who stick with you as you travel the rest of the Pokemon world. You can also have a duo join you as a single companion, but they're a package deal.
No you cannot have sex with any of them.
Example: I start in Paldea so companions are Iono and Sango. I travel to Sinnoh next and I replace Iono with Arezu (just assume she got time warped) and now I'm stuck with Sango and Arezu. Of course I cannot have sex with them.
