The island has most of the major urban development in the northeast with the capital and is more rural in the rest like the south and west, with a big mountain range in the center that you often pass through to get around
You'd probably start in a rural area and work mostly around the mountains to maybe a more notable city like Ponce, before crossing over/through them to San Juan later in the game, and the classic climax where you finally ascend the mountains you've been dancing around the bottom of through the game like in Sinnoh
There's a local legend about the time some british pirates were coming, so the women and children were sent up the mountains with torches to safety, but the british just saw the mountains blanketed in torches lighting up the night and assumed it was reinforcements coming from the other side of the island, and decided to not take their chances and noped the fuck out
This would probably be adapted to something like a legendary attacking and the torches from those fleeing being mistaken for the abilities of a powerful Fire-type or something, probably a hidden postgame quest involving lighting torches around the mountains to summon it. Off the top of my head, maybe some sort of guardian deity made of smaller flame spirits pooling together like a Wishiwashi or something
There's a smaller island to the east called Vieques, originally kept by the navy as a bombing/testing ground but people kept telling them to stop being fags and give it back and eventually they relented. Now it's part wildlife refuge, part off-limits due to contamination and unexploded ordinance. This would definitely be a postgame area, maybe housing the other legendary from the legend, and the off-limits areas are where it's scarred the landscape with its powers
