It's literally Pokémon Red & Blue but the assets are from Gen 4 HGSS.
The day care is a breeding center and bill's place has a trading machine to trade to yourself to evolve your trade evos.
The dex is the Kanto dex, but all the evolutions from generation 4 are available and you don't have to wait to post game to be able to unlock them.
The sevii islands aren't accessible but Birth Island is accessible once you complete the dex mew is there.
You get to rematch gym leaders and Oak is able to be battled after defeating the champion.
Each time you defeat the Champion you get to choose one of three rewards, one of the starters you didn't pick or the fossil you didn't pick are options, and by defeating the Champion three times you exhaust all the prizes and none remain.
There's also a day & night cycle.
Post game visit Bill and he'll be working on a time capsule which will malfunction and a spikey-eared Pichu will appear and you'll obtain it.
I think that's basically all the differences, obviously mechanics are gen4 as well. All bred moves that aren't obtainable are tutorable.
