Instead of being Carmine's younger brother, he'd be Onion (the idea being a Bombirdier got really drunk and somehow managed to swap Carmine and him at birth)'s older brother. But even with that change, I don't think it takes much to keep the plot of the Hidden Treasure of Area Zero on track.
There'd doubtless be less (fairly unnecessary) battles but the elements for Kiki to go off the deep end and return are all still there. The obsession with Ogerpon's strength, Briar being just the fucking worst etc. The details would change, with Kieran's inferiority complex turning into something of a big brother's arrogant seniority "how could gramps favour you over me and not tell me about the ogre, I should have been the one" etc. but I feel like it'd still work surprisingly well.
And as for Carmine's time in Galar... well, I feel like she'd end up even more vainglorious than she already is, using the Galar league's format and sponsors to inflate her head even bigger. Maybe a little clashing with Kabu, when she discovers she's actually from Kitakami and tries to use that as a gimmick to increase her own clout, kind of like a dark mirror of how the canon!Carmine is fiercely protective of her heritage, while this one commercialises it for her own benefit.
