I think Whitney's group commissioned that Gym themselves and eventually became "official" (the anime establishes that there's many more than just 8) when the Pokemon League took notice of them, due to Trainers showing up with their badge and talking about how tough/annoying Miltank had been. She's a gym leader because of her skill and Pokemon, not because of her personality.
