We literally have a canon version in the games and the anime, and they're the same. It's an alternate version of our planet where you can clearly see the continents.

And although they're no longer directly mentioned, South America, China, Russia, and other "regions" still exist, as mentioned in G1. They'll just change their names, as happened with America and Unova (This is hinted in bw2 with Surge)

I see little reason to believe they aren't. Other than minor differences in shapes and climates, which can easily be handwaved by the world not being a 1:1 copy of Earth, but just very similar. The regions are based on real countries, it stands to reason that the world is similar on a global level as well. It's the logical conclusion. I think we need more evidence that it isn't.

Perhaps they're in the approximate area that their real world counterparts occupy but in many cases the shape or size varies somewhat. For example, Unova is probably much bigger than it's real world counterpart.

Assuming Unova includes New York City and the surrounding areas, it would probably be similar sized or only a little smaller than Kanto and Johto, and slightly smaller than Hoenn, Sinnoh or Alola. It's really the regions based on entire countries that are the outliers, the regions based on Japan are small regions, and Hawaii isn't that big either.

Anon, many are almost an exact copy of their real counterpart, for example Orre in the first game seemed like a fantasy region, but then they updated the map and it's basically Arizona and part of California on the American side and Baja California, Sonora, and the Sea of Cortez on the Mexican side.

In the ReBurst manga, the unnamed region is a vast area with a highly populated area on the east coast, the canon region of the video game, and the rest is a vast Wild West-like region with small towns. It's hinted that it's still Unova since they only have G5 Pokémon.

But the manga failed, so I don't know if this part of the lore can be salvaged.
