given how they keep bringing Kanto back, which is based on an actual place by the same name, is there any reason to believe the world map would be completely different from the real world?

To add, I'd also say earlier gens were a lot less obvious about not being set in the real world. Kanto region being named Kanto and matching the irl part of Japan is one big example of that, but there's also pokedex mentions of real animals, Lt. Surge being called an American, mew being discovered in Guyana. They started retconning that in later gens.

It's definitely not a one-to-one map, the regions have their own distinct geography compared to their real-world counterparts, but I do think it's safe to say it roughly maps to the real world overall, at least in terms of where each region is in relation to each other. We know Unova is far from the Japanese regions, Galar and Kalos are within swimming distance of each other, Alola has close ties to Kanto and Johto, etc. So no, it really doesn't seem like the Pokemon world map would be completely different from the real one

I think that GF intended Pokemon to take place in a pseudo-real world at first. Also shown by mentioning South America in the Mewtwo diaries, or the Indian elephants in Raichu and Gastly's dex entries.
And then by gen 2, they changed their mind, and Pokemon has become more and more of its own world with each gen.

It's because you're using the old map, the current Orre is an almost 1:1 copy of Arizona, California (US) and Baja California, Sonora and the Sea of Cortez (Mexico).
