Are all regions oriented like the ingame maps or is it a gameplay thing entirely? Because Unova, Alola, and specially Hoenn and Galar are clearly rotated quite a bit

Only way to tell is if the game's map has the cardinal directions shown on it, or there are wild Nosepass that point north like in Hisui and Kitakami.

This gym distribution is crazy, I wonder how many gyms and leagues exist in the world, because the gym density changes a lot from one continent to other

It’s just a consequence of GF making entire countries into regions instead of actual regions within the countries. Galar at least has 18 gyms (in lore) but if I understand correctly there’s always only 8 of them that are part of the gym challenge.

I imagine that the general locations match up, like Alola in the pokeworld is 1:1 where Hawaii is. With some liberties taken with the physical structuring of the region, like Unova having a Desert and a volcano further NW of that unlike New York
