Basically the synapses spark to send a message through the central nervous system when it sees something it likes, the person then experiences that sensation in the brain - which results in arousal, either sexually or in a sensory way (e.g. increased saliva production, nosebleed, etc).
It's part genetics, and part of an experience that is had as a child (well according Freud that is), and creates a framework for what is perceived a fetish (or rather what is seen as taboo in public) going forward.
