So at the very least it's not diabetes from this one symptom, unless she's going to provide more symptoms to Dr. Anon Y. Mous for a better diagnosis.
It's most likely related to her gallbladder removal and is just a consequence (read: "have to live with it/change diet where possible"), as bile from the gallbladder is used in fat digestion, and the ammonia smell comes from the body struggling to digest protein (via not having enough fat to digest said protein from said gallbladder removal).

Ketosis is very bad for you, it can cause renal failure and is shown to damage your heart, brain and liver alongside the kidneys in prolonged states (ie "on a keto diet for more than a few weeks"); anyone who says a keto diet has no downsides is lying.
People in medicine are just as likely to be kooks and idiots as the rest of us in my experience.
I had a surgeon for a hernia that was convinced that dairy consumption (both in you and your mother while pregnant) was the cause for type-1 diabetes and tried to sell me on it when he *should* have been running through what my recovery should be like.
