They usually have bathrooms because of the sheer size of the store, and the ease of brownie points in the "family" market by letting a changing table be available to parents.
Wal Mart has bathrooms because it's a supermarket, and usually the ONLY grocery store in smaller towns. They have bathrooms because they're a do-all store; my local Wal Mart still sells guns 20ft from the Lego aisle, which is 20ft from the home goods (bedding, laundry, cookware, misc stuff like that), which is 20ft from the motor oil, which is 20ft from the cat food, which is 20ft from the pharmacy, which is 20ft from the electronics, and the other half of the 1500ft wide warehouse store is reserved for groceries.
American supermarkets are an experience I don't wish on anyone.
