Of course the very big ones must be (I believe?) making bank through donations or selling audios on DLsite or Fantia, but I have seen many who don't really average much more than a couple thousand views per stream and rarely break the 200 concurrent live viewers mark. I imagine those aren't really getting much out of it and probably keep a dayjob and do streams on the side. And if you look up their channel they have been streaming pretty consistently for months or even over a year.
How do they do it? What's their endgame? It's genuinely impressive desu
