Even if Mel did it accidentally, I could easily see it being severe enough or something she did repeatedly until it got to the point where Cover had to axe her. This is the first graduation/termination for Hololive where I'm not aware of what exactly caused it, so while I'm not happy I also can't really assume malfeasance on Cover's part.
