I've worked within a few different bloated bureaucracies and a lot of the time people just forget why procedures exist and follow them even when they serve no purpose. Then you end up with people who get obsessed with the "right way" to do something, who seem almost vindictive in their refusal to see why something may be unnecessary in the given situation.
