In 2021, Miko had to take 3 months off for surgery and she posted a diary entry with a drawing every day for her fans. When she got back, she cried and said how grateful she was that everyone stuck with her. When Kanata caught covid last year and had a 104+ temperature, she posted one short video a day for her fans. When Pekora took a month off for her voice a few months ago, she posted about her morning exercise and the activities she was doing each day. It was her first break that she had ever taken since joining Hololive three and a half years ago.
Aqua, Shion, and Ayame are pretty much on constant breaks nowadays, but even they still message their fans almost every day.
If the problem with Gura is strictly physical like you're suggesting, then there is zero reason to believe that she cares about us at all in any way whatsoever, beyond monetarily.
