Katawa Shoujo is a visual novel created by "Four Leaf Studios," a group of anons from /a/. The entire game was created by these anons, with the game based on a sketch by Raita Honjou from 2000.
The premise of the game is that you play as a boy named Hisao Nakai. The game opens with Hisao confessing to a classmate, who confirms his affection, which causes Hisao to suffer a heart attack. Following the heart attack, Hisao spends months in the hospital recovering (and receiving a diagnosis for arrhythmia, an irregular heartbeat), slowly losing his friends from his old school. Following the isolation and medical requirements, Hisao is enrolled at far-off Yamaku Academy, a boarding school purpose-built to accomodate students with disabilities of all kinds. Upon arriving, he is introduced to some of his classmates and other personalities around the school, 5 of which are the romance options in the game:
-Emi Ibarazaki (missing her legs after a car accident)
-Hanako Ikezawa (severe burns, scarring and PTSD from a housefire where she lost her parents)
-Lilly Satou (blind)
-Rin Tezuka (her arms are severely deformed due to a birth defect, so she effectively has no arms)
-Shizune Hakamichi (deaf)
The other characters are a supporting cast, the named ones that show up more than once are:
-Shiina Mikado (Shizune's translator, no medical condition confirmed otherwise)
-Kenji Setou (legally blind, Hisao's across-the-hall neighbor)
-Akio Mutou (Hisao's homeroom teacher)
-Yuuko Shirakawa (Yamaku librarian and waitress at the nearby Shanghai cafe)
-Akira Satou (Lilly's older sister)
-Hideaki Hakamichi (Shizune's younger brother)
-Miki Miura (left hand amputation, she is only interacted with in Hanako's route once, but was planned to be another romanceable character).
The game is a set of 3 branching paths, each featuring 2 girls, with each being broken down further depending on which choices you make in dialogues: Emi and Rin, Lilly and Hanako and Shizune and Misha
