Baphomet was a propaganda symbol created by the Knights Templar in the 1300's. He was invented to justify crusades against the muslims, as the Knights touted that Baphomet (a bastardization of "Muhamad") was the deity that Saracens/Muslims worship. He has no basis in actual Christian scripture, and the first mention of his name was in heresy trials held by the Templars themselves. This is why many depictions of Baphomet include the depiction of a Crescent Moon, a traditionally Islamic symbol.
While the origin Baphomet's visual appearance is unclear, its generally accepted that his appearance largely lifted from the Celtic Shaman God Cernuous (pictured).
