Basically she's saying her (and other holomen) made sure management knew mistakes like that definitely aren't okay no matter how busy they are, how there should be checks in place to stop things like that from happening.
She also said that during situations like this it's better to send the person who's affected by it kind words/praise instead of focusing on attacking the people who made the mistake, and that it's better to let the people directly involved in the situation handle it.
