By default MMD has one global light source, you can manipulate it in the green area. You can load a stage or other junk in using that tan area.
The "MMEffect" thing I have pointed out in red is something extra you have to install, then you can install Ray-MMD.

TL Note: MME is just a few files you drop in your MMD directory and that's it. Ray-MMD is WORK.
You have to set up to 50, depending on the model different materials for each model you import, although you can save effect profiles to save time in the future.
It's not always clear what each sub-material represents. Things like Roboco's different ears or Miko's different eyes all shrink and vanish inside their heads when not in use, so you'll spend 10 minutes messing around with a half-flayed model trying to figure out if you need to set a specific material option or not.
It also comes with its own lighting engine. You can add multiple light sources, environmental lighting, etc. That starry background I've been using just came with the skybox I loaded up the first time I used it. It won't work without a skybox loaded in, and once you've got Ray-MMD "active" on a given project, you can't export images (or video) with a transparent background anymore, unless there's some kind of way to force it to allow alpha on the "back" of the skybox.
