I've taken some time to process Shibethany's Elden Ring stream.

My reading is that this happened while Madam felt regret over neglecting her duties, returning to her Christian roots: self-punishment. She created a stage for said punishment. The sniffling was, if not out of allergies, because of her guilt and chat asking the questions physicians, policemen, teachers, friends ask.
Do you ask: "How does your mom dress before going to work?" if she's not suspicious?
It's in this sense, and the stream being unscripted, that the experience was realistic enough, even while fetishized by the characters and (part of the) audience.
At some point Shibethany asked if chat was feeling uncomfortable.
