Then it generates that image fully. Then it basically does an img2img pass on it. But the important part is that it is doing it from scratch again to add increased details. Depending on the noise setting you can get something completely different or something thats sort of similar but has increased details. If you are looking to just upscale your current gens I think there is a fork of the webui that adds the highresfix functionality to the img2img part of it.
