She asked if it was ok to laugh at a tranny joke through marshmallow because she didn't want to be cancelled, and when asked about trans people she said sure she's fine with them but has never met one and doesn't care about "political stuff". Likely her whole view of trans people as a British girl that doesn't leave her house is the crazy green haired schizo types.
She probably wouldn't pick up on someone being trans unless they told her and knowing Shondo's niceness I doubt she'd say anything mean regarding it. I wonder if deep down it'd make her feel differently though.
