That AI setup reminds me of Macross Plus, there's actually this scenario of an AI-assisted mech reading the thoughts of the pilot a bit too smartly and executing an offensive command that is what the pilot wants, but not what he would do. Though in that case it's officially blamed on a bug in the software instead of the pilot or the AI... If they're so effective the people selling and using that tech would have a pretty strong incentive to silence this kind of incident. (Also Macross Plus is pretty great, it does some things with human-AI relations that you might find inspiring, would recommend).
