They spent a long time building a safe oassageway for neuro to get into the stronghold (so she doesn't die from drowning) and the spot they chose to sit at had occasional guardians attacking them. Neuro's health was slowly getting chipped away over the course of several minutes and she asked for help multiple times. I don't think anyone realized how low her health was and she just got killed moments after Filian got into the stronghold.
