I just had this discussion with some people a month ago about whether it's better for you to eat throughout the day or have only one or two meals, assuming the same amount of calories consumed.
Turns out, it doesn't really matter. A few hundred years ago, most people only ate lunch and dinner. Before that, most people only at one meal in the early evening. And they were probably more physically active than most people today.
The worst issue is feeling hungry most of the time, and I imagine if you are used to one meal a day, your body adapts so as to not feel nearly so hungry most of the time. Assuming you keep to a semi-consistent time at which you eat the meal.
If they've confirmed he looks malnourished, then I could see there being issues.
