I was gonna say Challenger on launch, but I just looked it up and Soyuz MS-10 failed in October 2018. Had a booster failure and the launch escape system was trigger, and both astronauts were fine.

Yeah it's important to remember the space shuttles had no launch escape system. If a rocket does go boom now there's a way better chance of the crew's escape and survival.
