If both were to go out by suicide, Matsuri would be viewed as insane mental case trainwreck that destroyed and ruined her own life, and Rushia's suicide note would be the type that makes you puke out of how sad it is just reading it and wonder how fucked up they were in the head this whole time and survived this long. The majority of the "major issues" people you put are essentially having a rough episode mental case or bouts of major issues that can be viewed as the results of poor circumstances or curable eventually fixable after enough time passes, or have just one or two peculiarities fundamentally off about them that are extremely abnormal and schizo behaviors; but Rushia's is essentially a living mental issue, that's her normal state which has only gradually worsened in intensity over her entire life, the "fine" moments she shows to the world are abnormalities from her natural condition,. Even at the peak of what should bring her happiness, nothing is cured, it's just somewhat suppressed. Her livestream persona is thousand percents more mentally healthy than she actually is (mainly due to livestreaming being the only time she's happy), even her deleted menhera mengen posts as Rushia & deleted menhera tweets as Rushia are still fundamentally more sane and mentally well off than she actually is, and I think the schizo behaviors she displays on stream alone are on par with most the roommate issues of most the others in Major Issues. I could write a whole thesis on her mental state but that requires a lot of info I can't source on /vt/ and don't really care to write out.
But Rushia's just a fundamentally broken case way past the other members, a walking mental issue that's just really doomed to worsen as time goes yet more on, a mental sickness unto death, the menhera she portrays on stream is thousands times more healthy than her actual state of mind. I don't really see Rushia going any other way out except by suicide, even with her extremely frail constitution, it's still most likely Rushia ends up taking her life in a fit of despair. Other fandeads might deny this but that's their own delusions of thinking they can fix Rushia and wishing her happiness. I've witnessed her and her roommate enough to be able to know it's hopeless, the same way it's hopeless to save or fix Matsuri. Matsuri's more traditionally insane by society's standards, Rushia's just fundamentally broken.
