Has any holo ever had their avatar completely redrawn by a different artist? I mean, is it totally without precedent in the worst case scenarios?

Suisei, but the original was made by herself so dubious if that counts.
Sora and Miko both have current redesigned base models made by someone other than the original artist. Both are 3D talents though, not live2D.
Azki and Nene both have redesigned base models, but by the same artist.

If you included Nene's redesign then yes, it counts. Although Flare's case is more than a costume redesign, but it's not drastic enough like Suisei, AZKi, and Miko.

AZKi's 3 costumes had different designers. She changed to the 3rd artist because the 2nd passed away. 3rd artist recreated/updated the 2nd costume.
