The specific mistake isn't clear, but there are good guesses. It could be in the actual topology of the model's face, or it could be that they showed model from an angle that it isn't meant to be seen.
But if that's the case, you still have the question of who showed the model from the poor angle. Who decided where the camera was? Did they not tell Mori she couldn't show that angle? Did they tell her and she accidentally did it anyway because she's just not used to having to restrict her movement? Hard to say. Probably just caused by a young modeler not fully having mastered the anime face topology needed for lighting, which is pretty tricky as far as 3D modeling tasks go.
